Governed Decision Record
The sealed, tamper-evident record produced at the moment a consequential AI-assisted decision is made, capturing what was decided, under what authority, on what evidence, by which system, and with which accountable person.

Decision; authorized remit; model or agent; institutional context; standards evaluated; verification state; evidence provenance; accountable authority; timestamp; seal.
Defensible
A decision is defensible when its record can survive examination, by a board, counsel, a carrier, or an auditor, as it existed at the moment of decision, without reconstruction. Defensible does not mean guaranteed correct, risk-free, or a substitute for professional judgment; it means the basis, the authority, and the evidence can be shown.
Authorized remit
The documented scope of decisions, actions, and data a system or agent is permitted to operate within, declared before operation and accountable to named leadership.
Verification state
The recorded status of whether a system's observed behavior conformed to its documentation and remit, determined by behavioral testing rather than configuration review, and captured in the decision record at the time of the decision.
Evidence provenance
The traceable origin of every input a decision relied on: who or what supplied it, when, and under what authentication.
Accountable authority
The named human role responsible for the decision, recorded in the record itself. Systems inform; the accountable authority decides.
When verification fails
The decision path fails closed: the action is blocked or withheld, a sealed record of the failure is produced, and the matter escalates to the accountable authority. Absence of verification is treated as failure, never as permission.
